The router is the one AT&T gave us to use (sysinfo says NOKIA BGW320-505). When I first set up the plex server with remote access it worked fine. Then, at some point, it stopped working. To fix this first made sure to SSH in and update the server. Then I disabled/re-enabled remote access from both the plex settings and the router. Sorry but this is not a Plex issue, this is a router issue. UP&P is not reliable enough and most retail routers do a very poor job implementing it and it should be turned off anyway since it is a security issue. I don't know if its the same issue for you but its worth checking that you can reach the Amazon Web Services IP from your Plex server. I'm running pfSense and had to white list them in order for remote access to work.
